Indication

• Adjunct in treatment of alcohol dependence.
• Adjunct in maintenance of abstinence from opioids after opioid detoxification.

Action

Opioid antagonistIn alcohol dependence it reduces craving for alcohol and possibly reduces some of the pleasurable effects, by blocking the effects of endogenous opioids.In maintenance of opioid abstinence, reversibly blocks opioid receptors for (24-72hours) preventing opioid effects e.g. euphoria.

Dose advice

Alcohol dependence:

• 50mg once daily.Maintenance of opioid abstinence:
• Initially 25mg daily; if no withdrawal effects occur, use 50mg daily.

Common side effects

Usually transient, subsiding after 1-2 weeks of treatment:

• Nausea
• Headache
• Dizziness
• Anxiety
• Fatigue
• Insomnia
• Sleepiness

Uncommon side effects

• Hepatotoxicity (more frequent with doses >50mg daily).
