Primary Outcome Measures:

  • Measured at Months 6 and 12: Behavioural self-efficacy
  • Patient activation
  • Health behaviours, including exercise, physical activity, and smoking
  • Health service use

Secondary Outcome Measures:

  • Measured at Months 6 and 12: Health-related quality of life
  • Body mass index (BMI)

Eligibility & Criteria

  • Ages Eligible for Study: 18 years and older
  • Genders Eligible for Study: Both
  • Accepts Healthy Volunteers: No

Inclusion Criteria:

  • Currently receiving treatment at Dekalb Community Service Board
  • Chronic disease

Exclusion Criteria:

  • History of more than two psychiatric or medical hospitalisations within the year prior to study entry
