Action

Enalapril is converted in the body into enalaprilat which is a type of drug known as an angiotensin converting enzyme or ACE inhibitor.

Enalaprilat inhibits an enzyme called angiotensin-converting enzyme and by doing this produces dilatation of the blood vessels and increased salt and water loss from the kidneys resulting in a subsequent drop in blood pressure. Enalapril starts to lower the blood pressure within one hour after administration and has its greatest effects 4-6 hours after administration.

Dose advice

High Blood Pressure:

  • The recommended initial dose in patients not on diuretics is 5 mg once a day.
  • This dose is then adjusted depending on the blood pressure response.
  • The usual dose range is 10 to 40 mg a day given as one single dose or two divided doses.

High blood pressure:

  • The usual dose is 20 mg once daily.

Heart failure

  • The initial dose of enalapril should be 2.5 mg or less once a day.
  • If necessary the dose can be gradually increased over 2-4 weeks to 10 to 20 mg given in a single or divided dose.

Left ventricular dysfunction without symptoms of heart failure:

  • The initial dose of enalapril should be 2.5 mg twice daily.
  • If necessary the dose can be gradually increased over 2-4 weeks to 10 mg twice daily.
