Indication

Maintenance treatment in severe asthma and chronic obstructive airways disease

Action

Not entirely understood

Possible effects include bronchial smooth muscle relaxation; anti-inflammatory effects; increase in diaphragm contractility and CNS stimulation

Dose advice

Maximum
900mg daily for maintenance dosage
Adult / child >6 months with no risk factors for decreased theophylline clearance

Initial dose 10mg/kg daily; maximum 300mg daily; if initial dose is tolerated increase after 3 days

First increment 13mg/kg daily, maximum 450mg daily. If the first increase is tolerated, increase dose after 3 days


Second increment 16mg/kg daily, maximum 600mg daily. Measure plasma concentration after 3 days at the highest tolerated dose.

Controlled release preparations are given every 12 hours; rapid release preparations are given every 6 hoursElderly

Clearance of theophylline derivatives decreases with age. The elderly require a lower dose
