Indication

Prevention of long standing, stable angina.

Action

Glyceryl trinitrate relaxes smooth muscle throughout the body. Its mechanism of action is based on an increase in venous capacitance leading to decreased return of blood to the heart. This results in decrease in left ventricular end-diastolic pressure (preload), which leads to decreased myocardial oxygen requirement at rest and during exercise. It also causes redistribution of coronary blood flow to ischaemic subendocardium by selectively dilating large epicardial vessels.

Dose advice

Treatment should be initiated with application of one Transiderm-Nitro 25 system every 24 hours.

Dose may be titrated upward as follows:

  • one Transiderm-Nitro 50 system (normal maintenance dose)
  • one Transiderm-Nitro 25 system plus one Transiderm-Nitro 50 system
  • two Transiderm-Nitro 50 systems Tolerance is seen with frequent or prolonged administration of long-term nitrates.
  • Thus a patch-off period of eight to twelve hours at night every day is recommended.

Application:

  • Transiderm-Nitro system should be applied to intact, clean dry skin that is free of hair.
  • It should be changed daily to minimise skin irritation.
  • The application site should be changed regularly.
  • It should not be applied to the distal parts of the extremities.
  • In overdose, severe hypotension and reflex tachycardia or collapse and syncope can occur. Effect can be terminated by removing the system.
