Indication

Used for severe acute asthma
Used for maintenance treatment in COPD and severe asthma
Used as alternative to or in combination with beta 2 agonists in COPD

Action

Promote widening of airway diameter

Dose advice

Adult
MDI â?? 40 mcg, repeat 3-4 times daily as necessary; up to 80mcg 3-4 times daily may be needed
Neb â?? 250-500mcg up to 3-4 times daily
(dilute solution for nebulisation with 0.9% sodium chloride solution)
Child
MDI (child<6years) â?? 20mcg, 3 times daily
MDI (child 6-12 years) â?? 20-40 mcg, 3-4 times daily as necessary
Neb â?? 250mcg up to 3 times daily
(dilute solution for nebulisation with 0.9% sodium chloride solution)

Common side effects

Dry mouth
Throat irritation

Uncommon side effects

Urinary retention (there is a risk of aggravation of symptoms in patients with prostatic hypertrophy)
Acute angle closure glaucoma (there is a risk of aggravation of symptoms in patients with coexisting glaucoma)
Constipation
Allergic reactions (Ipratropium MDI should not be used by patients with allergy to soya lecithin or related food products such as soya bean and peanuts)
